Directions: This final report
plus the preschool evaluation form will be your final.
Therefore, take some time and effort. Answer all questions
using correct grammar, spelling, sentence structure,
as well as being well organized and indepth.
Describe your observations
of the child(ren) you were assigned in each of the following
areas. Make special note of any changes that occurred
from the first to the last week.
1. Physical Development
- include small and large motor.
2. Intellectual or Mental Development – memory,
language, sentence length, following directions, knowledge
of numbers, letters etc, concentration length etc.
3. Social/Emotional Development – If there were
times that these were separate then indicate the situation.
At this age they a closely related so I left them in
one category.
4. Other – This category includes anything that
doesn’t fit in to one of the other categories.
This could also include anything that you observed that
seemed to relate all the areas together. Another item
that might be considered here would be any observation
of moral development – evidence of knowing right
from wrong and following through with the “right”
behavior.
As you write the observation
report be sure to clearly label each section –
do not mix them all together. This report is graded
on whether you know each type of development.
